Social Workers

At Vision On Care, our professional Social Work team provides high-quality, person-centred support to help participants create positive change in their daily lives and enhance their personal and social wellbeing.

Our social workers work closely with participants to identify challenges, develop strategies for improvement, and connect them with the right community, housing, and support services. We also make referrals to our trusted network of providers to ensure each participant receives the most effective and appropriate assistance.

Tasks & Responsibilities

• Provide practical support, counselling, and emotional guidance to participants and their families.

• Offer accurate information and advocacy, ensuring participants’ rights are protected and their voices are heard.

• Assist participants in overcoming barriers related to housing, health, education, or social inclusion.

• Act as an advocate for clients experiencing inequality, injustice, or discrimination, helping them access fair treatment and equal opportunities.

• Collaborate with other professionals and community organisations to deliver holistic, coordinated care.
