Support Coordination
Vision on Care has clarified a support coordination group that will work with you professionally to understand and implement the fund and all categories in your care plan. Also, this team is being given full authority by an organization to link our participants to the community, mainstream and other government services as well as full support and build your skills and direct your life.
- Conduct a needs assessment with the participant, based on their NDIS goals and the level of service defined in the NDIA plan.
- In concurrence with the participant and their family where appropriate, develop a support plan that clarifies the actions to be taken and outcomes to be achieved.
- Provide services and maintain written program documentation as outlined in service policy and standards guidelines, consistent with the Disability Act and the NDIS act.
- Participate in the development of new services as required.
- Action referrals in a timely manner.

Support the NDIS participant to:
- Assess a number of mainstream, community, informal and provider options.
- Select preferred options or providers.
- Negotiate services to be provided and their prices.
- Develop service agreements and create service bookings with preferred providers.
- Arrange for any assessments required to determine the nature and type of funding required (e.g., assessment to determine the type of complex home modifications required).ferred providers.
- Determine the budget for each support type and advise any relevant plan manager of the breakdown of funds.
- Liaise with any plan manager to establish the appropriate claim categories and attribute the correct amount of funds.
- Provide to the NDIA timely and relevant progress reports reflecting progress towards achieving plan goals, and recommendations about the appropriate type of support required in the next plan and reasons for this.
- Link to mainstream or community services (i.e., housing, education, transport, health).
- Strengthen and enhance participants capacity to coordinate supports, self-direct and manage supports and participate in the community, including providing participants with assistance to - resolve problems or issues that arise - understand their responsibilities under service agreements - change or end a service agreement.
- Be the key contact for service issues, complaints, major changes and plan reviews.
- Maintain confidentiality of customers, their families/carers and the organization.
- Effective networking internally and externally to assist individuals to achieve their desired personal outcomes.
