Support Coordination

At Vision On Care, our dedicated Support Coordination team works closely with you to help you understand, implement, and make the most of your NDIS plan.

Our coordinators are experienced professionals who will guide you through every step of the process connecting you with community, mainstream, and government services, while helping you build the skills and confidence to direct your own life.

Our Support Coordination Services Include:

Needs Assessment: We conduct a detailed assessment with each participant based on their NDIS goals and the level of support outlined in their NDIS plan.

Individual Support Plan: In collaboration with the participant and their family (where appropriate), we develop a clear and tailored support plan that defines the actions to be taken and the outcomes to be achieved.

Service Delivery & Documentation: We provide services and maintain all required documentation in line with our service policies, the Disability Act, and the NDIS Act.

Program Development: Our coordinators actively contribute to the development and improvement of new services to better meet participant needs.

Timely Referrals: We ensure all service referrals and linkages are managed promptly and efficiently.

Through our Support Coordination service, we empower participants to take control of their NDIS journey, strengthen their capacity, and achieve greater independence.

How We Support NDIS Participants

At Vision On Care, our Support Coordination team is dedicated to empowering participants to make informed choices, build capacity, and take control of their NDIS journey. We provide hands-on guidance to ensure every participant understands their plan, connects with the right supports, and achieves meaningful outcomes.

Our Support Coordinators Will Help You:

Assess Options: Explore a variety of mainstream, community, informal, and provider options that best meet your individual needs and goals.

Select Preferred Providers: Assist you in choosing the most suitable service providers based on your preferences and support requirements.

Negotiate Services and Pricing: Help you negotiate service terms, pricing, and inclusions to ensure transparency and value.

Develop Service Agreements: Support you in creating service agreements and making service bookings with your chosen providers.

Organise Assessments: Arrange any necessary assessments to determine the type and level of funding required. For example, complex home modifications or specialist equipment.

Manage Budgets: Determine the budget allocation for each support type and communicate this breakdown to your plan manager where applicable.

Coordinate with Plan Managers: Liaise with your plan manager to establish appropriate claim categories and ensure funds are attributed correctly.

Provide Progress Reports: Submit timely and accurate reports to the NDIA outlining your progress toward achieving plan goals and recommending any adjustments or future supports.

Link with Community Services: Connect you with essential mainstream or community services such as housing, education, transport, and health.

Build Capacity: Strengthen your ability to coordinate supports, self-manage your services, and participate confidently in the community.

❍ Assist in resolving issues or disputes that may arise.

❍ Help you understand your rights and responsibilities under service agreements.

❍ Support you in changing or ending service agreements when needed.

Be Your Primary Contact: Act as your main point of contact for any service-related issues, complaints, major changes, or plan reviews.

Maintain Confidentiality: Ensure that all participant information, including details about families and carers, is treated with complete confidentiality.

Foster Collaboration: Build strong internal and external networks to help you achieve your personal goals and desired outcomes.
